    Information for Eevee's Transport

    Prior to Transport

SAMPLE PAGE FOR DISPLAY PURPOSES ONLY! DOCUMENTATION AND REQUIREMENTS ARE DIFFERENT FOR EACH TRANSPORT ARE DIFFERENT. YOUR PERSONAL PAGE WILL REFLECT WHAT IS NEEDED FOR YOUR TRANSPORTASAP
Please take the time to fill out our Contract for services
    and our Emergency Authorization Form
Eevee will need to be Microchipped prior to any of the below. If she is already Microchipped, please make sure that is recorded on all forms for transport.Forms Needed For Transport:
    (2) Valid Rabies Vaccine Certificates
    USDA Endorsed Health Certificate
    Rabies Titer Test
(A titer test is not needed for CR, but included in Sample Page as many International Countries do require it)
    Details for each are below
For your <insert country name here> International Destination, Eevee will need:ASAP
    - A valid, current Rabies Vaccine Certificate, with microchip number, date given, brand, lot number, expiration date listed and signed in blue ink. If you do not have this, contact your vet, they should be able to reissue one for you. PLEASE NOTE: Most countries outside of the US do not recognize the 3 year vaccines, so if Eevee has a 3-year, it will only be recognized as a 1-year. So if she is beyond that 1 year, she will need a new rabies vaccine given.
    - A booster Rabies will be needed if there is a lapse in Rabies vaccines within the past 3 years, or if any of the rabies were given prior to Eevee being Microchipped.ASAP - Nov. 20, 2025
    - If Eevee is current on her Rabies, it is time to get a Rabies Titer Test done. This test shows how many rabies anti-bodies Eevee has, think of it as a test to prove that she has her rabies vaccines and they are working. Your local vet should be able to do the blood withdrawal and send it to an USDA Approved Lab (see list here)
Please email a copy of these results as soon as they are received
    Dec. 15, 2025 early morning
Vet appointment with an USDA Accredited Vet for Eevee's USDA Health Certificate. During this exam, Eevee will need internal (de-worming) & external (flea & tick) parasite treatment administered and documented on the Health Certificate. The Vet will send in the Health Certificate for USDA Endorsement. Once endorsed, the USDA will email a copy back and send a physical copy overnight (you will need to provide postage for this). We will need this by Friday for transport on Monday, so if there is no updates by Wed noon, please let me know so I can reach out and see what is going on.
PLEASE email me a copy of all paperwork prior to Saturday, Dec. 20 so I can prepare extra copies needed for transportSunday Night, Dec. 21, 2025
Please pull the Eevee's food at midnight prior to the morning of travel and limit water intake. This will help her feel more comfortable (and clean) during the flights, and help ward off any health issues that could arise from Eevee "holding it" to long, as most cats are prone to do in new situations. I will provide a secure pet playpen and portable litter box when possible. Food and clean, filtered water as we get closer to our destination.
